Orthopaedic Manual Physical Therapy
In the routine management of musculoskeletal conditions, we utilize skilled passive movement to joints or soft tissues for the purpose of reducing pain, improving mobility, or enhancing muscle function. These procedures encompass an entire approach to musculoskeletal physical therapy that range from examination to intervention. Based on individual patient presentation, we utilize the full continuum of intervention techniques ranging from non-thrust to thrust joint mobilization. Wise PT therapists have received extensive training and have provided training for other practicing therapists in this area of specialization. They are certified in manual therapy and have also attained the distinction as Fellow of the American Academy of Orthopaedic Manual Physical Therapists, of which there are only a select few in the state of Pennsylvania. Wise PT therapists have also achieved Board Certification and re-certification in orthopaedic physical therapy from the American Board of Physical Therapy Specialties.
Low back pain impacts an estimated 80% of all individuals at some point in their lives and 25% of all individuals who seek physical therapy care do so because of neck pain. We specialize in the management of spinal impairment syndromes ranging from acute to chronic conditions. We develop a collaborative relationship with our patients that is uniquely designed to meet each patient's specific and individual needs. We routinely utilize pain relieving modalities, therapeutic exercise, and skilled hands-on care that is designed to reduce pain and improve function. Wise PT therapists have been involved in teaching both entry-level and practicing therapists in the care and management of patients with spinal impairment. They have also presented and published research in the area of spinal rehabilitation.
Therapeutic Exercise to improve mobility and muscle function
Mobility as well as muscle strength and endurance can be enhanced through the use of specifically prescribed exercise. We closely examine each patient and construct an exercise regimen that meets each person's individual needs. In order to achieve maximum benefit, close attention is given to the manner in which these exercises are performed. In order to empower each patient toward maximal function, we provide an exercise regimen and instructions for ongoing progress into the future. Wise PT therapists have been involved in teaching entry-level doctoral physical therapists in the principles and practice of therapeutic exercise for over 10 years.
Pain is the primary reason why most individuals seek outpatient physical therapy care. We use procedures that are specifically designed to reduce and control pain. Modalities may be used to both prepare and support the patient for interventions that are designed to address the primary origin of the symptoms and make lasting change in the patient's condition.
Edema reducing interventions
Edema or swelling is a normal physiologic response to acute injury. It demarcates the initial phase of the healing process. However, edema may result in a reduction in mobility, muscle function, and may cause significant pain. When edema persists or leads to functional limitations, we use a variety of procedures ranging from hands-on care to specifically-designed equipment to reduce edema and restore function.
Sports Injury Rehabilitation
Involvement in sports often leads to injury and inability to return quickly to pre-injury participation. We closely evaluate the demands and requirements of each sport and target the patient's program accordingly. Since no two sports are the same and no two patients are exactly alike, no two programs should be the same. Each program is designed to meet the unique demands of the sport while appreciating the specific nature of each injury. Specificity of training and a complete understanding of the process of tissue healing is critical for the safe and timely return to sport. Wise PT therapists are also certified athletic trainers by the National Athletic Trainers' Association and have been involved in providing rehabilitation for both recreational and competitive athletes for over 20 years.
Rehabilitation and Reconditioning for the Injured Worker
The longer an individual is out of work for a work-related injury, the more likely they are to stay out of work. For individuals who have been out of work for greater than 6 mo's, only 25% return to the same job and for those out of work for greater than a year, only 50% return. Whether an injury has occurred from work-related causes or whether an individual's non work-related injury is preventing them from returning to work, we appreciate the time-dependent nature of these conditions. Being out of work translates into a significant financial hardship for many individuals and a challenging situation for most employers. As your advocate, we collaborate closely with each injured worker to gain a complete understanding of the critical demands of each job. We target our programs to return each person to the highest level of employment through work re-conditioning and simulation. Timely communication is critical and we advocate, on behalf of the worker, to employers and physicians about each worker's safe return to gainful employment.
Foot Orthotic Management
Persistent foot pain may lead to pain and disability. When structural impairments impact normal foot function, functional foot orthotics may be indicated. We perform a detailed lower extremity examination and casting which results in functional foot orthotics for a variety of musculoskeletal conditions impacting children and adults. We appreciate the impact of the entire extremity on foot function and work toward improving motion and strength deficits that can be changed prior to orthotic prescription. Our objective is to eliminate the need for orthotics, if possible. In addition to effective orthotic prescription, each patient is educated in proper foot wear and an exercise regimen designed to maximize the effects of the orthotics is provided. Our orthotics are custom-molded by a local orthotic fabrication lab based on the measurements and specifications that we provide. They come with a lifetime guarantee on the orthotic shell. Wise PT therapists have been prescribing functional foot orthotics for hundreds of patients over the past 20 years.
Headaches are a common problem that significantly impact an individual’s ability to maximally function at home and work. Many people are not aware that a large majority of headaches may come from mobility restrictions or soft tissue compromise of the neck. We utilize specific exercises designed to reduce stress and improve mobility in addition to hands-on soft tissue and joint mobilization techniques. Educating individuals in proper posture and daily exercise is often useful at controlling symptoms into the future.
Many individuals complain of the acute onset of dizziness or vertigo that may profoundly limit their ability to perform even basic activities such as getting up from lying or walking down the aisle at the grocery store. Many conditions producing such symptoms come from impairments of the inner ear. Wise PT therapists will work closely with your physician in providing the most appropriate care so that you can return to living a “balanced life”.
Injury to the wrist and hand complex may pose significant challenges to normal function. The anatomic complexities of this region make rehabilitation challenging. Whether the condition is related to an overuse injury or a significant traumatic event requiring surgery, Wise PT therapists will provide appropriate care designed to facilitate healing and normal function. A variety of splinting options designed to protect and promote function will be explored as needed.
Pain, edema, limited mobility, and reduction in strength are common in the post-surgical patient. The nature of the surgery often dictates a long recovery time and gradual return to function. It is critical that during the immediate post-surgical phase appropriate care is given that exploits the window of opportunity for achievement of optimal function without interfering in the normal process of healing.
